INITIAL TRIP REQUEST: Zak is five and has neurofibromatosis (tumour behind the eyes) that is effecting his sight. He is the youngest of three children and Mum is on parenting payments through Centrelink.

This Angel Flight was for Zak to undergo a MRI scan as part of the medical and surgical consultation at Westmead Children's Hospital.
